Completed Reservation Request Form and Program Contract with Signature All This form is intended for use by solar professionals to get the signature of their clients at the time of sale.

A Reservation Request Signature form can be submitted in lieu of the Reservation Request Form.

Print this form and obtain required signatures. Then proceed to the next page in PowerClerk and electronically attach a scan of the signed form.
Documentation of an Energy Efficiency Audit if you have not met Title 24 or other exemptions. Title 24 documentation is mandatory for non-residential new construction. All All applicants are required to have an energy efficiency audit conducted. A customer can, however, be exempt from an audit if documentation is provided which shows that an energy efficiency audit was completed within the past 36 months, the facility is Energy Star or LEED certified, or the facility is Title 24 energy efficiency compliant within the past 3 years. See Section 2.3 of the CSI Handbook for more details.

Copy of Executed Agreement of Solar System Purchase and Installation All All Applicants must submit a copy of an executed agreement to purchase and install the solar system at the time of submitting the Reservation Request Form. This agreement must also include warranty language to meet the warranty requirements. This agreement should be provided by the Solar Contractor. Scan and upload PDF copy of agreement to PowerClerk. Make sure the host customer and the contractor sign the contract and that the CA license is included.
Authorization to: Receive Customer Information or Act on a Customer's Behalf CSE Applicants Only This authorization is specific to CSE territory applicants only. Authorization to Receive Customer Information Print this document, sign, scan and upload to PowerClerk.
Certification of tax-exempt status (gov't and non-profit only) Government, Non-Profit and Public Entities only Any Government or Non-Profit entities must include a certification under penalty of perjury from their chief financial officer or equivalent that they are a Government or Non-Profit entity and that the system is not receiving, and will not in the future receive, federal tax benefits through financial arrangements (i.e., the System Owner if a third-party, which will be receiving tax benefits from the system). This certification must be renewed annually if receiving PBI payments. This documentation should be provided by the Host Customer's Chief Financial Officer or equivalent. Electrical System Sizing Documentation (new/expanded load for systems greater than 5 kW) Systems greater than 5kW with new/expanded load This is documentation to confirm that participating solar energy systems will not exceed the capacity of the Host Customer's previous 12-month historical usage. All Applicants for projects at new construction and/or major renovation must submit a copy of the data and calculations used to determine electrical system size.

Copy of Executed Alternative System Ownership Agreement (If System Owner is Different from Host Customer) If System Owner is different from Host Customer If the System Owner is not the Host Customer, the applicant must submit a copy of Executed Alternative System Ownership agreement. Copy of Signed Commitment Agreement (EPBB Existing Commercial buildings ≥ 100,000 sq ft and Benchmarking < 75) (Proof of Benchmarking Documentation req'd) EPBB Existing Commercial buildings >= 100,000 sq ft and Benchmarking <75 In order to qualify for a CSI Incentive, retro-commissioning is required if an existing commercial building has a benchmark rating of less than 75. A Commitment Agreement must be completed and signed by the Host Customer (for buildings with a benchmark rating less than 75) along with proof of benchmarking. Proof of at least 15% above current Title 24 Standards (for non-residential new construction only) New Construction Non-Residential only Compliance documents used to demonstrate Title 24 compliance, including the PERF-1 form and accompanying supporting forms, shall be provided as proof of attainment of the Tier I or Tier II levels. Compliance documents shall be completed by persons who are Certified Energy Plan Examiners (CEPE) by the California Association of Building Energy Consultants (CABEC). Building Site Plan (non-residential new construction only) New Construction Non-Residential only The Applicant shall provide a site plan that for each lot identifies the height category (small, medium, or large) of all pre-existing, planted, and planned trees and the location and height of any structures which will be built on the lot, and neighboring lots, of the building with the solar energy system; and shows the bearing of the property lines and the azimuth and tilt or roof pitch of each PV array. Copy of New Construction Building Permit (non-residential new construction only) New Construction Non-Residential only The new construction building permit must be submitted in order to identify applicable Title 24 requirements. High Cost Justification and Acknowledgment Form For Applicants whose proposed project costs exceed the limitation on installed costs, as defined in Section 3.4.5 of the CSI Handbook. This form provides documentation to the Program Administrator that the particular project requires a special configuration that will drive project costs above the limitation on installed costs. See Section 3.4.5 of the CSI Handbook for more details on limitations on installed costs. High Cost Justification and Acknowledgment Form Print this form, provide justification for high cost of project, obtain required signatures, scan and upload to PowerClerk.
Documentation of CPUC Code 2852 eligibility All MASH Demonstrates that the project site meets the low income housing definition per Public Utilities Code 2852. Examples of such documentation are: Executed Regulatory Agreement, Deed Restrictions, Affordability Convenants, etc. This should be provided by the Host Customer. Virtual Net Energy Metering Solar Allocation Form (MASH) MASH applications interconnecting under MASH Virtual Net Energy Metering Provide the solar allocation sheet from the Virtual Net Energy Metering Interconnection Application. The solar allocation sheet indicates how solar energy credit will be allocated among the benefitting accounts within the MASH low-income site.

Floor Plan and Sq Footage Calculation or Other MASH applications for master metered buildings only This document identifies the common area and tenant unit allocation for master metered buildings. For floor plans, common area and tenant square footage must be clearly identified on the plans. If floor plans cannot be located, an on-site energy analysis by a third party can be submitted as an alternative. If submetering data is available, the file must be from the third party submetering company and should identify the site address(es) of the submetering account(s). Completed Proof of Project Milestone Checklist All This checklist is to assist in the completion of information materials required for review of Proof of Project Advancement and to speed processing of applications. This form can be retrieved from the Proof of Project Milestone page in PowerClerk. Print this checklist and obtain required signatures. Then proceed to the next page in PowerClerk and electronically attach a scan of the signed form.
Copy of Executed Agreement of Solar Energy System Purchase and Installation All Applicants must submit a copy of executed contract for purchase and installation of the system, and/or alternative System Ownership agreement. Agreements must be legally binding and clearly spell out the scope of work, terms, price, and solar energy system components to be installed. Agreements must be signed by appropriate parties (Supplier/Solar Contractor, Host Customer, Applicant and/or System Owner). This agreement should be provided by appropriate parties (Supplier/Solar Contractor, Host Customer, Applicant and/or System Owner). Scan and upload PDF copy of document to PowerClerk.
Copy of RFP or Solicitation (gov't non-profit, and public entities only) Government, Non-profit, and Public Entities Only Government, non-profit, and public entities must submit a copy of the Request for Proposals (RFP), notice to Invite Bids, or similar solicitation issued for the installation, lease, and/or purchase of the system proposed for the Project. The RFP or solicitation must include sufficient documentation details including the scope of work, schedule, terms, budget, and system components to be installed. Incentive Claim
Completed Incentive Claim Form with Signatures All This form is intended for use by solar professionals to get the signature of their clients in order to officially request disbursement of the incentive. This form can be retrieved from the Incentive Claim Form page in PowerClerk. Print this form and obtain required signatures. Then proceed to the next page in PowerClerk and electronically attach a scan of the signed form.
Signed Field Verification Certification Form For Applicants whose Reservation Request Application was first received on or after 7/1/09 The Solar Contractor must perform a Field Verification prior to submitting the Incentive Claim Form. The Field Verification Certification Form must be signed by the contractor. Signed Field Verification Certification Form The form must be submitted to the Host Customer and a copy retained by the contractor. The form may be requested by the Program Administrator at any time after an Incentive Claim Form is submitted.
Copy of Executed PDP Contract (PBI Only) PBI Systems Only All PBI systems are required to have a Performance Data Provider (PDP) and therefore a copy of the executed contract for a PDP, a letter from the PDP stating the Host Customer has purchased its service, or an invoice from the Solar Contractor clearly showing the PDP information must be provided. Whichever document is submitted must clearly identify the PDP information, including the name of the PDP, the product or service purchased and the term of agreement, along with the address of the associated solar energy system site. PMRS Cost Cap Exemption Documentation (if no eligible PMRS is installed) If no eligible PMRS is installed For all PBI systems, it must be indicated on the CSI Incentive Claim Form that a Performance Monitoring and Reporting Service (PMRS) is installed and name the eligible PMRS provider. Systems receiving an EPBB incentive must also indicate the PMRS provider on the Incentive Claim Form or submit PMRS Cost Cap Exemption Documentation if no eligible PMRS is installed. The Cost Cap Exemption Documentation consists of any of the following items that demonstrate PMRS costs exceed the cost cap:
  1. A quote from an eligible PMRS provider indicating the PMRS provider's cost for providing the basic PMRS described in Section 5 of the CSI Handbook; or
  2. A quote detailing the equipment, installation, maintenance, and five year service costs of any communications equipment and service required for the provision of the PMRS (if such equipment and service does not already exist at the Host Customer premise); or
  3. An invoice or quote detailing the associated metering system costs (if separate from inverter and only if necessary for the provision of the PMRS); or
  4. A letter on the Solar Contractor letterhead showing any additional costs, including labor, materials, overhead and Solar Contractor mark-up, to install and maintain the PMRS.

Copy of Retro-commissioning Report (EPBB Existing Commercial buildings ≥ 100,000 sq ft and Benchmarking < 75) EPBB Existing Commercial buildings >= 100,000 sq ft and Benchmarking < 75 for Reservation Request Applications first received on or after 7/1/09 A copy of the retro-commissioning report is required for EPBB Existing Commercial buildings ≥ 100,000 sq ft and Benchmarking < 75, as represented in the Commitment Agreement. In order to qualify for a CSI Incentive, retro-commissioning is required if these existing commercial buildings have a benchmark rating of less than 75. This should be provided by the Host Customer.
